“Treatment resistant” is one of those labels that can quietly steal hope. When a treatment plan fails, it is easy for the system to shrug and imply the patient is the problem. We sit down with Dr Jacob Silverstone, Medical Director of Rewired Ketamine in Miami, to ask a sharper question: what if the care pathway is what needs to change?

Jacob brings an unusual perspective, moving from reconstructive surgery and clinical research into ketamine therapy after seeing how many people struggled post-COVID with depression, anxiety, PTSD, suicidal ideation and chronic pain. We talk through what evidence-based IV ketamine looks like in real clinic life, from loading doses and session structure to “guiding” without turning the experience into psychotherapy. He explains why his team designs a calm, non-clinical environment while keeping the medical underpinnings strict and safety-led.

We also tackle the myths head on: fears about danger, the “horse tranquilliser” headline, and worries about losing yourself. Jacob compares ketamine’s risk profile at sub-anaesthetic doses with medications many patients receive routinely, and he shares which presentations tend to respond well and which ones remain difficult, including severe OCD and certain personality disorders. We finish with the uncomfortable business realities: commercialisation pressure, investor algorithms, and why even educating people can be blocked when “ketamine” is a restricted advertising term.

Guest Biography

Dr Jacob Silverstone, DPM | Medical Director, Rewired Ketamine

Dr Jacob Silverstone is a US-trained podiatric physician, former reconstructive foot and ankle surgeon, clinical educator and Medical Director of Rewired Ketamine, based in Aventura, Florida.

Before moving into ketamine-based care, Jacob held senior clinical leadership and educational roles, including Division Chief and Residency Director for Foot and Ankle at Mount Sinai Medical Center. His professional background also includes involvement as a principal investigator in pharmaceutical research and procedural innovation.

His interest in ketamine developed through his surgical and anaesthetic experience, eventually leading him to establish a practice focused on patients experiencing persistent mental health difficulties despite conventional treatment.

At Rewired Ketamine, Jacob advocates an individualised approach that combines medically supervised treatment with guided sessions, psychological support and behavioural integration. He has a particular interest in questioning established treatment pathways and examining how clinical care can better accommodate patients whose needs are not adequately addressed by conventional approaches.

He is also the author of Chronically Misunderstood and Stuck in Reverse: Rewiring Your ADHD Brain.
